Thanks for clarifying. That's not the default so it sounds like you have
enabled:
org.gnome.desktop.wm.preferences focus-mode 'mo

I have never had any gnome extensions installed.
Just to be clear, window focus appears to work as it should for all
windows, i.e., focus is gained as soon as the cursor enters any portion
of the window.

Please try removing all local extensions:
cd ~/.local/share/gnome-shell
rm -rf extensions
and then log in again. Does the bug still occur?
