I can confirm this bug. I am in Karmic with a Macbook 2,1 and the
headphone jack does not work. The alsamixer workaround did not work for
me, and I see no way to unmute surround, though I'm fairly certain I did
in Jaunty before I upgraded to Karmic.

I don't know if the problem is coming from alsa or the mixer.
Maybe someone could try to install an old version of booth to try.

Where can I find it? There is no Gnome mixer in Karmic as it was in
previous releases. Console alsamixer doesn't show the Surround item as
it was showing it in Jaunty.

I've tried following instructions from
https://help.ubuntu.com/community/HdaIntelSoundHowto
but it didn't help.
I've checked the documentation:
/usr/share/doc/alsa-base/driver/ALSA-Configuration.txt.gz
/usr/share/doc/alsa-base/driver/HD-Audio.txt.gz
/usr/share/doc/alsa-base/driver/HD-Audio-Models.t
Me too. Workaround worked in Jaunty. After upgrading to Karmic, sound no
longer comes out of headphones, and it is impossible to follow the above
workaround instructions.
