Gergely is correct. You must place such preseeded values in the initrd.
See section B.4.1 of the installation-guide.

You cannot use preseed *file* to change keyboard layout since these questions
appear *before* CDROM even accessed.
You can use kernel parameters tought. Try:
append console-setup/ask_detect=false console-setup/layoutcode=us
..
in isolinux/isolinux.cfg file.
