It also happens with metacity themes as I have just discovered.
If you cannot read the theme it reads 'gaia siberia' and can be downloaded
from here:

1) Yes, it is reproducible, see attachment above.
2) Install the theme above, and you should get two entries for the same theme
in the theme manager.
3) I am running 8.04, but I believe it has occurred in previous versions too.

It only appears to happen with certain themes, no way to know without
actually trying them, the DarkIce theme in the original screen shot does
cause this.
