I had the same problem.
Mine started when I did Open With -> kpdf
(Sorry can't remember why I did that, it was working fine before)
Anyway I couldn't follow Drew's description after "If we go back to the
file associations list via Konqueror (mentioned earlier) we can see KPDF
listed but if we hit
Should be fixed in KDE 4.1

The same problem with kdeinit/kpdf happens to me. It seems that all
begun after installing acroread. Now I purged it, but the problem is
still there.

I just tried it with a clean install of Kubuntu Hardy, beta1, kde3.
fully updated as of today.
I ran konqueror, went to
located pdf, deleted all program associations, clicked on a pdf file to open
it, picked kpdf out of the list and checked "Remember application
association..."
kpdf opened as
Thanks for submitting this problem and helping to make ubuntu better.
I have followed those steps on hardy (with the exeption that I used
okular instead of kdpf) and the problem did not occur.
Could you please test this on hardy.

Also tested it with kpdf. The template is not empty, put fills in
correctly. No problems opening the pdf file.
