This is more a bug in Gnome sound settings than in Totem since it uses
the default Alsa card but Gnome doesn't change it. This problem has
already been reported. Marking as a duplicate.

I have the same on a Xubuntu 7.10 machine. I solved using gxine or xine-
ui, changing default soundcard to "plughw:0,0" inside the preferences
section. No chances for totem-xine, anyway, and starting it overrides my
changes as well... :(
