I was able to partially work around this:
1. open KDE's systemsettings app
2. select "Keyboard & Mouse" > Mouse > Cursor Theme
3. select a DIFFERENT cursor than the current (desired) one
4. click Apply
5. re-select the desired theme
6. click Apply again

I'm having this problem in Ubuntu 10.04. It appears in the program Anki,
which uses QT. I think I've installed this version from the author's
website.

For someone looking for a solution to this, run kde
configuration(systemconfig or kde-config) and change mouse pointer theme
there and it will take affect for qt apps.

In Ubuntu 8.04 I set the cursor to DMZ-Black. And then I run lyx, which
uses Qt. After lyx starts, the cursor resets to te default cursor. I
looked the theme and it says it's still DMZ-Black but it isn't. Anyway,
I set the cursor to show DMZ-Black again but moving the pointer to the
lyx application
Is there any update on your issue? What Ubuntu are you using?
