I've been successfully using the mythtv_alsa_non_mmap_fallback.2.patch
from http://svn.mythtv.org/trac/ticket/3598 for a while with PulseAudio.
So far so good.

Indeed it does. Problem exists in 0.20.2, patch applies cleanly, and
corrects problem.
