I can make this error happen on demand - I try to connect to a Windows
share, with an incorrect domain, get an error, the top menu bar with the
screen minimize, maximize and close goes gray and I can not click on any
of the minimize, maximize or close buttons.

If you have visual effects enabled, disable it and enable again. Then
upper menu-toolbar must appeare.

you sure you didn't remove the bar from the configuration by mistake?
