My apologies, this seems to work ok. I think my issue was that it seems
to take a moment to gather CPU usage information.
For the first few seconds, all processes show as zero, but they
eventually do show real percentages.

I have this problem in Hardy, which has gnome-system-monitor 2.22.0. If
this was fixed in 2.19.x then why is it still an issue?

Just a note: One my other Gusty machine, one with a Intel Pentium 4 640
instead of the AMD Athlon 1100+ I posted above, it has the same problem.
So if it helps, it doesn't look CPU related.
