I don't know what additional information to supply. I'm seeing this
form because I tried to upgrade Ubuntu, and the upgrade crashed and
asked if I wanted to report the bug. Here's what I see in my terminal:

I'm seeing this bug in 12.04 LTS. The date at the top of the dropdown
is always Tuesday, 1 January 1980. But the correct date is highlighted
in the calendar, and the date command gives the correct date.
