I have the exact same problem with an Intel 945GME (rev03) and an Acer monitor
connected via vga-out.
The highest resolution xrandr shows me is 1152x864, tough the monitor is
capable of 1280x1024.
Turning the monitor off and on and restarting X helps, but only until

If you want to continue to use hardware mixing, you don't really need to enable
software mixing (dmixer). Softvol alone is sufficient.

I had a look at the 2.6.17 kernelsources and possibly found the reason why
these modules are not included... It's necessary to enable the (deprecated)
"Video for Linux 1"
interface in the kernel configuration, then you are able to select the PCI
cards.
Please do so!

Since the upgrade to version 0.10.12 (ubuntu edgy BETA) Banshee crashes when I
try to import music to my collection. I deleted all my settings
(~/.gnome2/banshee) and tried it again, but the error still occures!

After doing a software suspend ("standby") my soundcard doesn't wake up.
Running alsamixer & co works but all players i tested (totem, mplayer, xine)
say "can't connect to audiodevice".
In the kernellog I found following line:

To make one master-fader out of the two DAC-outputs (left and right) I
enabled software-mixing in addition to the softvol-plugin:
