Reproducible with LO 4.4.0.3, Win 8.1.
This is really not intuitive if you are not used to it. The currently
available online help does also not help. These options should be explained in
the LO help. I for instance do not have any idea what "calc8" means.
Is it also maybe possible to add too
I tested it with LO 4.4.0.0.beta1 (Win 8.1) with EXPORT AS PDF and with
PRINT -> pdf printer.
-> I could not reproduce it as it is seen in the pdf attachment (but I
don't see the logo in the top left).

If I open the attached file "Original.doc" with LO and save it again as
a doc file and afterwards reopen it with MSO 2007 and LO, then in both
programmes the heading is missing. In this case I can confirm a missing
header with LO 4.0.2.2 (Win7 Home, 64bit).
