No doesn't happen in Lucid for me, fresh install. Yelp very slow though,
grays out unresponsive frequently, enough for me to ignore it now and
just use man pages from a terminal or Google if I want to know "howto"

Had the same issue, although "update-grub" found the correct file; the
image wouldn't display in the grub menu. My fix involved an edit in Gimp
to re-save the image in RGB mode, NOT grayscale or indexed. It works
with *.png and *.tga files. This is referenced in

Yes had the same problem after deleting a partition. BasicWolf's solution
worked perfectly.

Bug is/was constant, happened every time.
However, On removal of Firefox 3.5.1 along with 3 versions of xul runner,
replacing with a fresh Firefox 3.0.13; along with removal and reinstall of Yelp
+ it's deps, the problem seems to have been cured.
The 3.5 version of Firefox was insalled alongside
