I can confirm this bug. I tried kernel 2.6.21-14, -15 and -16, and
linux-firmware 1.26. My difference is that manual loading of modules
don't solve the problem, I had to connect using Ethernet. My machine is
a Dell M6400, with an Intel Corporation Wireless WiFi Link 5100

Marking as invalid since - as you wrote - it was an volume related issue and
not a kernel/driver bug.
I didn't checked alsa because I didn't touched low-level tools willigly to
avoid messing with pulseaudio.
Perhaps pulseaudio brought both volumes to zero crashing sometime ago and I
didn't notic

On a Dell Precision M6400, with an Intel 82801I (ICH9 Family) HD Audio
Controller (rev 02) audio is hearable on the speaker but not on the hearphones.
There is also a periodic high-pitch audio "blip" that is always audible either
from hearphones or buitin speakers.
