I'm observing this on Xenial, although I've never previously experienced
it. I've been using Terminal every day on Xenial for a few months with
no issues.

window decorations don't appear
I believe something was corrupted while switching drivers. It's possible
that the control center tool that does software updates and changes
proprietary drivers hit some issue and was not able to do an atomic
rollback. I am not sure what went wrong, however, since the workarounds
I linked to above
