I had a ghost monitor indeed. Per's workaround from bug 2060268 worked
to eliminate the ghost monitor:

I believe this is the simpledrm driver not asking for the conflicting
framebuffers to be unregistered.

The solution of Per Overgaard also eliminated the ghost monitor for me -

After applying the solution, I also had to blindly apply my disk encryption
password -

I usually have my display resolution set to 2560x1440, on my LG Electronics 27"
monitor.
When I set it to the original resolution of 3840x2160, the glitch disappears.

I noted that my headset also "glitches" with my phone; The music stops
for a few seconds ~ every 10 minutes. However, my phone seems to recover
this issue and continues playing the music after the "glitch".
