thanks for this thread! In my case it was the wrong css_styled_content Version, too.
After removing CSS Styled Content TYPO3 6.1 I reloaded my site and all pictures were found.

yeap i have the same problems, typo3 version 6.2 adding a image to context, in the backend i can see the image, but after showing front, only a big picture with a ? is showing. that is strange.
you r so lucky the problems dispeared automaticly, but i have the same problem all the time.
