I just "printed" the pages to a file in Acroread... don't know any
other way. With ps, there are some command line programs (ps2ps,
psmerge...) to do that, so maybe try converting the pdf to ps...?
The quality isn't as great, though.

Hmm, load it up in ghostview or gv (if you have a recent ghostscript),
select the pages and save them?
