When using STUN, I can detect my NAT type. The SDP contain x-NAT field (0:
unknown, 1: full cone, ..., 6: symmetric) which tells Kamailio the NAT type
of clients. Why doesn't Kamailio use that ?

the software architecture you mentioned would scale or even work in the
first place. Here is why,
1. You are registering same user + 1 times,
so if you have two asterisk behind kamailio then a single user registers on
bot
AFAIK after 1) Jitsi should send subscribe message to xcap defined list.
After this kamailio can send individual subscribe packets in backend.
But as you described - Jitsi does not send this magic subscribe. Problem
application is Jitsi - not kamailio.
I spend couple of weeks diving into xcap/rls
