myisam tables.

You could ofcourse look at dbmo
If You are using the acc module, then You could close the open calls in
the database, when kamailio has been killed. You should then use the
database of couse.

a statefull proxy and I all ready have dialog module working too but with
no database.

Killing kamailio doesn't end running calls. So if you restart kamailio
before a BYE, accounting will be correct on teardow
