Kamailio would be looking in the location table for "username='1001' AND
domain='198.23.139.21'"
You should check the Kamailio logs for what is happening when Asterisk
sends it the INVITE for 1001@198.23.139.21

Having said that, if you make a change in FPBX but have not yet clicked on
the "Apply changes" (which writes out the .conf files) your database data
might be out of sync with Asterisk. Probably not a huge issue.

Your video1_sipregs table does not seem correct.
my use case would not exceed 1,000 so having the
registers sent to all asterisk servers made sense. For a large scale system
I would keep registrations off Asterisk completely.
