So in my setup where FreeSwitch and Kamailio sit on the same physical
server:
Kamailio - Routes traffic to asterisk boxes
Freeswitch - SBC and RTP traffic B2BUA
Question is, can I load balance my voice traffic to the asterisk servers?

URI is not touched. The R-URI is however (there's a prefix added). When the ACK

can you see, why the Kamailio is returning a "500 Server internal
error"? This response must be either created by some module or by some
upstream server (e.g. in case of loadbalancing).
Kamailio itself would not create such response, you should (if at all)
find the answer in the logs...

if you have to deal with multiple asterisk boxes, it is better to use
permissions module to match the IP addresses of asterisk boxes
(eventually mapping them to an ID via the tag value) and dispatcher
module to send the traffic to them.
