I also built the pdf documentation and found that the ref manual was
essentially empty. (17 pages all title page etc)

Which pdf are you talking about? The reference manual is build in separate
sections.

yes, I was thinking about doing just that. Exactly like it is already
done for the /doc/index.html page.

In SageMath verison 6.6 command line it works as expected:
