On the first page you ask for a name, is that necessary?! Some questions
like the first one might be better if there is a list of possible answers to
choose from. That makes it easy to generate summary graphics.

What I meant by private and anonymous is that even though the
information you provide me could very well be published I won't
publish your name and it should not be easy to link individual
responses back to individual respondents.
