This is usually because the NAS-Identifier or NAS-IP-Address attribute in the
radius packet does not match the IP address of the Client clause.

to double-check Simultaneous-Use. Perhaps you do not have a reverse DNS
for that NAS?
Where OnlineDatabase is the Identifier for my SQL SessionDatabase.
