Sounds like you have QS scanning both incoming and outgoing email in
/etc/tcp.smtp. If you do not need to scan outgoing email for spam try
resetting its queue from:
.:allow,RELAYCLIENT="",QS_SPAMASSASSIN="on",QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue.pl"
to:
.:allow,RELAYCLIENT="",QS_SPAMASS
