Thanks for all the suggestions. It turns out rolling back to MySQL
5.0.x solves the problem. I guess I'm affected by something in 5.1 -
just don't know what.

Do recipient verification on your secondary MX.
Or even better, don't use a secondary MX. Real servers sending to you,
will try again. If you expect to have more than four odd days at a time,
you have bigger things to worry about anyway.

From what you've said so far it sounds more likely to be a forged
return-path/from, in which case adding and checking against spf records
would solve your issue.

Sorry, I misread your log. from=<> usually means a bounce of some kind -
could be legitimate. You'll have to see where the original mail was
sent to, the headers in the mail will give you a clue.
