You want to download the zip file distribution of php from php.net.
That will have an ext directory in it just like your current install of
php.net.
Grab php_ldap.dll from that zip file.

I already explained this error to you once, the MySQL server is
rejecting your connection. You need to talk to the person who set up the
MySQL server and have them allow connections from your host.
If you are the admin of this MySQL server, install phpmyadmin and figure
it out.

Mysql can be configured to only allow connections from certain hosts,
and it appears your host is not allowed.
You need to talk to the mysql admin.
