1 - It unpacks the PHP in C:/PHP
2 - It enters in the IIS
3 - Enters in properties of your website
4 - In ISAPI filters add "C:\PHP\php5isapi.dll"
5 - In Base Directory => Configuration => Add:
- Exec: C:\PHP\php5isapi.dll
- Extension: .php
6 - Ok... Ok.. Ok...
7 - It creates the folder

I've had the same issue on my IIS setup... its really odd however if you
click back and fourth on the executable path and the extension box a
couple times some of the buttons suddely becomes clickable again.
could you post a screen shot? if so I might be able to offer more exact
help.
