I'm not sure if these options should be set, because they are required only
if PXE server is on different host from DHCP server, which is not the case
for Fuel.

Thanks for reaching out.
As I was adding the contents of ml2_conf.ini to reply to you, I noticed
my missing configuration for [ml_type_vlan] for the network_vlan_ranges
I placed that in and re-popluated the database and I can at least now
receive a ping reply from the instances.
Run "apt-cache policy fuel-ha-utils" and see from which repository does it
come from, if your mirrors configured correctly, the package should come
from your mirror.
It also shows what command it failed to run, "/usr/bin/apt-get -q -y -o
DPkg::Options::=--force-confold -o APT::Get::AllowUnauthentic
