Hi All,
After few more days of checks I couldn't resolve it. I think I tried
pretty much everything.
I tend to think that the problem is CentOS/RHEL 5.x and the following error:

You were correct, I had to manually delete the incorrect endpoint
templates directly from the db and reapply, but once done it all works
sweet.

Did you file the blueprint under Launchpad ? I see no link on that wiki
page. You can reference the wiki page in the blueprint (and the other
