Still seems to happen for me with okular 0.15.70 on kde 4.8.97 (that I think is
rc2) using the project-neon package build 2+git20120721+r4029-1~quantal1; this

This *might* be a form bug; it's pretty consistent across Okular, evince and
Chromium.
(Chromium is slightly different, it gives the tick boxes a default of No which
makes it easier to avoid)
