I have a brother printer and the duplex-settings work for basically all
applications that I tried except Okular since being ported to Qt 5.
So it worked with Okular from KDE 5 when it still was a KDE/Qt-4-based

Maybe Okular just fails to provide the required info to the printer driver
which then prints using its default settings for duplex printing?
This could explain the apparent contradiction...
