We couldn't reproduce it reliably, but it happens rather often on the

Thanks for your replies. None of the theories match, so not sure what is going
on. I’m going to change the list configuration to not require implicit
destination, and perhaps some other reason for moderation will show up for
these emails. Will reply if we learn anything more a
