No, iTunes doesn't need to be open to play content. There's a setting in the
remote app that lets you maintain a connection to which ever thing you're
connected to. Also, once you have the remote app connected to your apple tv,
there's a control tab, that will let you actually control the ATV menus

I have noticed that there are several media streaming devices from
manufactureres other than Apple. Some of them have IOS apps that let you
use an Iphone or, presumably, an Ipod touch as a remote control. Does
anyone on the list know if any are accessible with Voiceover?

Before I started using the apple TV today, I used WD mybook duo. They make
it in 3TB and 6TB sizes. Not only can you stream media to your IOS device,
but you can control it with the WD2go remote.

First,as mentioned by Daniel, make sure that all Libraries on your Macs and
ATVs have all been added to your Home Sharing network. I'm guessing that this
has most likely been done since you have been using your Apple TV to access
content from your Mac.

It depends on what media you're trying to access. If you wish to access the
media within an iTunes Library that resides on one of your Home Sharing
computers, then iTunes on that computer must be turned on. If you simply wish
