In Mail, go to the Window menu, then the Previous Recipients item. In there is
a search field and a list of addresses; find the one you want gone and hit
delete. There's no confirmation, it's just gone.

Now, whenever I try to send to that list, the +subscribe is always added to the
"to" field and I can't figure out how to get rid of it permanently. It's
probably something very simple, but I sure can't find it.
