the fault is to do with the speech server.
If you have as an example, settings enabled to announce background app
messages, etc, the speech server trips your reading text. this can sometimes
hang. the only way to handle it as you say is to quit and restart voiceover.

This happens to me occasionally in TextEdit, and I hadn't noticed it until the
last update. I think I should let Apple know about this, just in case they
don't know. I think it has to do with VoiceOver somehow because if I turn off
VO and wait a few seconds and then turn it back on the

I have seen this in a couple of the apps i use. How often does this happen?
For me, it doesn't happen often, but when it does, I have to press comand f5 3
times to get VO to come back.
