This is a known issue with the Launch system services database. If you have the
maintenance utility called cocktail, you can go to the system tab then to
databases and have it rebuild the launch services database. This will
temporarily take care of the problem but it will come back and have

is supposedly a way to clear that, but of course, I can't find the article I
thought I had bookmarked that talks about it.
