Try the following:
1. With the Mail message open, interact with the body of the message, and
navigate to the attachment.2.With VoiceOver focus on the attachment, press VO +
Spacebar. This will open a shortcut menu, with several options for accessing
the attachment.

could you be a little more specific? I know about quick nav. are you saying you
turn it to links and then what? click on it or open it with the up and down
arrow keys?

that normally brings up a menu to where you can open or save. It will normally
open up in Preview. And sometimes text edit.

It usually will say, has attachment or attachments. At least, from my
experience, it does. You can mess with the filter options from the View menu
where there is an Attachment, option, but I don’t know how that will effect
your messages.
