The command to read from top of document is VO-b. VO-w is the command to read
visible text.
You can find all the VO commands by doing VO-h-h.

VO[w reads the entire thing, vo-a reads from the cursor to the end. Remember
that interaction matters; if you hit vo-a but are not interacting with the
document text, the whole thing gets read no matter where the cursor is.
