The number of users is immaterial. Sometimes, you may need to uncheck, then
recheck the box to ensure that it is in the correct state. ET has kindly
directed you to the necessary location in System Prefs.

Hello,
I have a log-in screen on my computer. However, Voiceover does not
automatically turn on on startup. I looked under VO settings and it
appears that the box is checked. The log-in screen has 2 profiles,
mine and guest user, could this be an issue?

For speech at login go to the System Preferences and then Users &
Groups. Near the very bottom you'll find a 'click the lock to make
changes' button which you will then authenticate with your Mac's admin
password. Once that's done you can interact with the "Users, Groups and
Login Options" list
