Did you press any button on the phone to make it work?
I had a similar problem yesterday but when i pressed play on my headset Vo
started talking in the headset.

I did that with the iPad. But from the iPhone, dead silence. Either
a reboot fixed it or removing the headset fixed it, can't remember now.

Try this plug your headphones in and hit up volume until you hear it.
Sometimes I have to do that with everything I plug in .
There are more volume adjustments and some are only from device not ap.

Just upgraded iPad and iPhone. iPad is fine. On the iPhone though,
Voiceover is not coming through my headset. I may have missed a message
here on how to remedy this but cannot find it. Thanks.
