Just a late follow-up, thanks a lot for providing your experiences with track
changes and comments. I’ve spent a fair amount of time experimenting with this
and I believe I have figured out how to reliably read and navigate through
comments. It works just as you say; I’ve set hotspots

Here’s a quick explanation of tracking changes with VoiceOver.
Turn on Track Changes in the Edit menu.
A Change tracking and annotations toolbar will appear.
This contains the Previous and Next buttons and tells you how many changes have
