There’s a pane in System Preferences called Startup Disk where you can choose
which disk your Mac boots from. You can find it through Spotlight Search as
well (do a COMMAND+SPACE, enter “startup”, and you should be taken right to the
result you want). You will then be in an accessible dialog box
If you have Voiceover enabled to start speaking at login, that is
what will happen.
Same with NVDA, if its set to start when you boot into Windows, it
will start speaking at startup.
When using Bootcamp, the two OSs do not interact. Its either OS X or
its Windows.

Ok, I understand you have to restart and then decide which OS you want, but can
I wait until NVDA or VoiceOver comes up, or is it just a matter of going
through the motion with the keyboard commands that we know? Speaking of the
keyboard command, what is it again please?

You cannot switch between these two screen readers when using Boot Camp because
Boot Camp only allows one operating system to be live at any given time. You'll
either be using voice over we're NVDA but you won't have the option to switch
between the two.
