I've had these problems since updating. Also when in safari on occasions, VO
crashes and I don't know what to do so I just shut down or turn voice over on
and off. This doesn't happen a lot but there are occasions since the update
and there is no pattern or warning when it's going

I'm googling this issue for you, and it's not looking pretty. I'll send you a
page off list and will keep looking. You may need to take this up with an
Apple rep from what I'm reading. But, I'll keep looking. (not time to worry
yet.:)

you might have to put in the install disk, restart, turn on voiceover in that
install session, then run disk utility and run the first aid on that disk.
