Since there is no guarantee that the user will be logged in automatically, trying to open the run dialog might not work. WINDOWS+ENTER is guaranteed to work unless something goes wrong, so it is the better option for starting Narrator. Also, there should be no need to hit ENTER every few minutes

Hi Marshall, I unfortunately can't answer your second question, but I can answer the first. You can install the windows machine in VMWare Fusion with no sighted assistance required. It will walk you through the setup process, and then it will take a while, maybe about half to an hour to complete.
