I have to disagree with Alex on the insert key functionality.
My system which I believe is default has insert-1 as next link insert -2 as
previous link insert-3 as next-unvisited link
insert -4 as next header
and insert -7 as next input field item.

I have this late 2011 Macbook Pro 13 here with no num pad, and I can use
VoiceOver absolutely fine, especially on El Capitan when I can now use either
or both the caps lock and control-option keys as VoiceOver modifiers.

Well that depends on how you use the keyboard.
There is a numpad commander so that the numpad keys acts as navigation keys
for Voiceover.
For example you navigate with numpad 6 and 4, 2 and 8.

You can't use the insert key as a screen reader modifier, only control/option
and caps lock. You can assign tons of commands to the num pad keys, but even
with that option open to me on my wired keyboard here, I never really use it. I
simply find it easier to have my commands on the main keyboar
