All of my settings were properly set, I have reported the issue to
Apple, and they have responded as they were able to recreate the issue
so I guess it will go on to the list of things theu will hopefully be
fixing. I was not able to fix the problem by restarting, but I did
toggle

Have a look in VoiceOver Utility in the Navigation category and make sure that
Initial position of VoiceOver cursor is set to Keyboard focused item.

You answered my first question with respect to confirming your web navigation
settings in VO Prefs. My next suggestion is to look in the Tabs pane of Safari
and make sure that the checkbox for making new tabs and windows active when
clicked is checked.

This is something that just started happening over the weekend, it has
happened to me before and it is quite frustrating as when I am using
safari any time I open a new tab I must first hit VO shift down to
start interacting with tab instead of automatically being taken in to
the new tab. Not sure
