The shift key actually does work, it’s just that Keystroke (customizable by the user preferences) expects you to use the correct shift (i.e. the left shift for keys in the right hand and the right shift for keys in the left hand). Press COMMAND + , (command comma) to bring up the user preference

Hello, I’m aware that there are many people looking for an accessible typing program for the mac. I’m proud to say that today the agency I work for (The New Mexico Commission For The Blind) has released a cross-platform typing program called Keystroke. You can download it from our websi
